Why join the practice?

Have you ever desired to see the same physician for all your medical needs and concerns?

We understand that each patient is different and deserves individualized care. Our medical practice meets family healthcare needs in the community with affordable primary care. Patient health and quality medicine is paramount to our practice and we pride ourselves on providing our patients with fully comprehensive and customized treatment options for a variety of medical needs from routine wellness exam and sick visit to care for chronic conditions and women’s health.

What services are covered in the membership?

The monthly service fee is your payment for ongoing primary care services offered by your physician, Dr Lena. These include comprehensive physical examinations, medical management, minor office procedures, care coordination, referrals to specialty care (when needed) and treatment plan.

Do we take Insurance?

No, we are a membership-based primary care practice that does not take insurance.

However, having insurance can help pay for services not covered by the primary care membership such as emergency room care, in patient hospitalization, specialist visits (e.g. dermatology and cardiology), radiology and lab work.

What if I have no healthcare insurance?

No problem. You can take advantage of our at-home medical services for your primary care needs and enjoy huge savings.

How do I pay for my membership?

We offer two distinct service models and payment options for your convenience.

Pay-As-You-Go

This option is for paying for each well and sick visit. It is comparable to walking into a clinic, just without leaving home! View our services, and book an appointment today!

Subscription Plans

These plans include unlimited well and sick visits in the comfort of your home for a low monthly cost. You also get direct contact to your physician, via phone, patient portal, text or schedule a telemedicine visit and  after-hours excess accessibility.  It's all included in your plan! 

Patients join the practice membership plan by paying a one-time family registration fee. To enroll, complete the Member Enrollment Form.

How do I join the practice?

First, complete the Member Enrollment Form. Once completed, Dr. Lena will reach out to you to schedule a complimentary Future Patient Chat. During this time you will learn more about us, and hopefully determine if our practice is the best fit for your family.

Should you choose to become a member after the Meet & Greet, we will set up your membership so you can quickly enjoy the benefits of being a part of our practice!

How do I reach Dr. Lena after hours?

Easy, reliable access: We are here when you need us! You can contact Dr. Lena directly via the patient portal, text, phone call, or video chat. No answering services!

Same day/next day appointments are available and included in the membership. Avoid urgent care, and ER visits (where applicable, as some patients may require emergent medical attention and intervention).

Can I cancel my membership?

Yes. This is a monthly membership with no long-term contracts, all we ask is a 30-day notification regarding your desire to cancel.
